The purpose of the requirement is to provide medical, dental case management, behavioral health services, and medical administrative support staffing in support of the Individual Medical Readiness (IMR) soldiers in the Maine Army National Guard who require case management services to resolve issues that are making the soldier a non-deployable asset. The requirement is for one lead non-clinical case manager (RN III), two care coordinators (Medical Record Technicians), and one Behavioral Health Case Manager. The contractor shall provide all personnel, equipment, tools, materials, supervision, and quality control necessary in support of the IMR.
The period of performance shall be for a base year plus one option year.
