This requirement is to establish a Blanket Purchase Agreement (BPA). BPA Calls will be delivery periods in which the Contractor shall provide Dry Ice cut into blocks 5x10x3/4 inches in polyester bags, with a maximum of 400 lbs each week. Contractor shall provide a maximum of 20,800 lbs of dry ice per Fiscal Year. Delivery will be made to the Department of Pathology, Tripler Army Medical Center. Shipping Lab, Room 2G-008, MCHK-DP every Monday morning NLT 7:00am or on the first working day of the week. Please provide pricing per pund of dry ice as well as the delivery charge per weekly shipment.
