CHARLESTON CMOP Statement of Requirements(SOR) Brand Name Request of part # is TPA1214.75025, Sharp Auto Mailer Bags OBJECTIVE: To reorder additional Sharp Auto Mailer Bags, part # TPA1214.75025. These bags have been used during the testing phase of an ongoing contract for a new Automated Pharmacy Fulfillment System(APFS) located at the Charleston CMOP in Ladson South Carolina which has taken longer than originally expected. No substitutions are being permitted at this point in the contract to help maintain continuity during the ongoing testing phase of the contract. HOW SUPPLIES SHOULD BE FURNISHED TO THE CHARLESTON CMOP: Delivery: The Charleston CMOP requires the delivery of 108 cases every Friday for 12 weeks of Sharpe Auto-bag. Any vendor submitting a quote will be vetted to ensure they have the ability to provide uninterrupted source of supply to support the Charleston CMOP. Administration: All deliveries must be accompanied by a packing slip that must contain the following information below as a minimum: Name of contractor Contract number Purchase order number Model or part number Date of purchase Case Quantity, unit price and extension of the item Date of shipment Delivery Location/Info: 1. Deliveries shall be delivered to 4136 Carolina Commerce Parkway, Ladson SC 29456 2. Due to the weight and crushing abilities, the cases shall only be stacked #3 cases high. 3. All cases must be palletized on a standard pallet and be shrink wrapped tightly to avoid shifting within the truck s trailer. 4. Every delivery must be delivered before 2:30pm eastern standard time to avoid paying overtime to unload the trucks. 5. All cases must ONLY contain the 1500 bags per case with no overages. Auto-bag should contain at least 25% recycled material Finishing: Bags must have .125 thick side seams for durability. Heat seal above the skirt &horizontal perforation between bags to help separate during packing. Vent holes: ¼ round punched thru vent holes to allow air to escape in bottom corner of bags Corner notches: Notches in the side seams to help break perforations.
