B.4 STATEMENT OF WORK SCOPE: Department of Veterans Affairs (VA), Orlando VA Healthcare System in establishing a fixed-price purchase order for Luxor LX3 with Q-VUE Ophthalmic Microscope and NGENUITY 3D Visualization System Brand Name or Equal to maximizes visibility during any type of ophthalmic surgery. The contractor shall provide all labor, materials, equipment, tools, and supervision requirement for the acquisition of the Ophthalmic Microscope. SPECIFIC REQUIREMENTS: To procure an Ophthalmologic Microscope LuxOR LX3 Q-VUE Brand Name or Equal Medical equipment for Surgical Service Department in the Orlando VA Medical Center. The vendor shall provide one (1) set of user manuals and technical manuals. SIGN-IN PROCEDURES All Contractor workers are required to sign in and out at the VA Police Dispatch at the corresponding locations as directed by the VA POC or designee at each facility. A valid state driver s license or state identification card is mandatory for all employees to have access to these facilities. All contractor employees are required to wear the assigned VA badge always. Everyone entering the OVAHCS facility is screened and visitors are limited. Face coverings are mandatory. Anyone with any symptoms of COVID-19 are not permitted inside the facility. WORK HOURS Normal business hours are 7:30AM to 4:30PM Monday thru Friday excluding Federal Holidays. Work completed outside this time must be requested through the COR. Requests for afterhours work must be submitted in writing to the VA POC two (2) weeks prior to work. The VA requires that information submitted must contain extent of work, workers involved, the affected areas, and the estimated times of operation. Subject to COVID-19 restrictions and approval.
