STATEMENT OF WORK FACILITY SAFETY ANCHOR RECERTIFICATION/ LOAD TEST VAMC MARTINSBURG Federal OSHA requires that all roof anchors to be used with Rope Descent Systems (RDS) be re-certified every 10 Years. Contractor will test and give stamped and sealed Letter of load testing results by a Professional Engineer with the State of West Virginia PE stamp. All testing will be in accordance with OSHA 29 CFR 1910.140, 1910.66, and 1910.26. Contractor to provide all parts, tools, equipment, and transportation necessary to complete the requirements for certification of 16 anchor points located in/on the water tower at the Martinsburg VAMC. Review all existing drawing available for system compliance Load testing of all available equipment as prescribed by the Engineer of Record Inspection of all available equipment Make recommendation for any remedial work that may be required. Contractor will co-ordinate scheduling and other requirements with the FMS Shop Forman Contractor is responsible for meeting all local, state, and federal regulation and laws as they pertain to completing this requirement
