SOURCES SOUGHT NOTICE TITLE: Medical Air System Service Contract BACKGROUND INFORMATION: The VA Loma Linda Medical Center is equipped with a Medical Air System that consists of (1) Powerex Compressor (model number MSQ15B54P5), and (2) Nano Air Dryers (model number NLD-120). The Medical Air System requires bi-annual and annual maintenance performed by a certified technician as recommended by the manufacturer. OBJECTIVES: The purpose of this contract is to have a contractor furnish all labor, materials, supplies, tools and qualified personnel to perform the required bi-annual and annual maintenance on the Powerex Compressor, the Nano Medical Air Dryers. The contractor shall perform maintenance on the listed equipment as recommended by the manufacturer s service intervals. The contractor is required to have an outside company certify the Medical Air System according to National Fire Protection Association 99, Health Care Facilities Code. The maintenance performed shall include the following: Replace Belts Air Filter Changes Check Amps and Volts Replace Dryer Inline Elements Replace Inline Element Drains Blow off coolers with compressed air CO Calibration Safety Control System Check Differential Pressure Check Inspect Coolers Inspect Dew Point Sublet Certification LICENSING REQUIREMENTS: Offeror must be certified and possess a current ASSE 6040 (American Society of Safety Engineers) Maintenance/ Medical Gas Certification. Offeror must possess a 30-hour Occupational Safety and Health Administration (OSHA) safety card. SPECIFIC REQUIREMENTS: Schedule work with the Program Point of Contact (POC) or Designee (48) hours in advance prior to start of work. Work timelines may be subject to change due to VALLHS operational requirements. Contractor must be able to perform work with no additional charges within (30) days of contact by the designated VALLHCS Contracting Officer Representative (COR). All personnel shall have a valid Loma Linda PIV flash badge prior to job start. Prior to working, ALL energy sources (electrical, water, etc.) shall be locked and tagged out, and upon completion of work, will be re-energized and verified. Workers or a designated lead or supervisor are/is to check in with the Graphics Control in the plant prior to work each day. Contractor shall conform to the following standards, policies, regulations, governing agency, and any other local, state, or Federal guidelines that are not listed here: OSHA Regulations, www.osha.gov NFPA, www.nfpa.org Contractor will coordinate with the local VA Loma Linda Health Care System GEMS Coordinator. Contractor is responsible for clean-up, safe removal, and off-site disposal of items used during the project in accordance with all OSHA, GEMS, VA, etc. This includes, but is not limited to, clean up materials used during the project, discarded and non-compliant doors, door parts, etc. Contractor shall perform work in accordance with existing, relevant standards and codes to minimize the possibility of injury to personnel, damage to government property/equipment. Contractor will not damage existing infrastructure and/or healthcare system property/spaces (e.g., surfaces, stairwells, corridors, panels, walls, elevators, service areas, floors, carpet, road, pavement, etc.), including but not limited to, duct work, door frames, doors, electrical work, conduit, low voltage conductors, piping, CAT V cable, drywall, flooring, ceiling, plumbing work, lighting, and/or blocking ingress/egress routes. Damage to government property may result in additional action. Every effort will be made to safeguard and care for all human life and property. The contractor will provide all required licenses, certifications, permits, documents, as required and requested. NOTE: Acceptable proof of vaccination includes a signed record of immunization from a health care provider or pharmacy, a copy of the COVID-19 Vaccination Record Card (CDC Form MLS-319813_r, published on September 3, 2020), or a copy of medical records documenting the vaccination. If unvaccinated, contractors shall show negative COVID-19 test results dated within three calendar days prior to desired entry date. Test must be approved by the Food and Drug Administration (FDA) for emergency use or full approval. This includes tests available by a doctor s order or an FDA approved over-the-counter test that includes an affiliated telehealth service. Documentation cited in this section shall be digitally or physically maintained on each contractor employee while in a VA facility and is subject to inspection prior to entry to VA facilities and after entry for spot inspections by Contracting Officer Representatives (CORs) or other hospital personnel. Documentation will not be collected by the VA; contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information. Contractor employees are subject to daily screening for COVID-19 and may be denied entry to VA facilities if they fail to pass screening protocols. As part of the screening process contractors may be asked screening questions found on the COVID-19 Screening Tool. Check regularly for updates. Contractor employees who work away from VA locations, but who will have direct contact with VA patients shall self-screen utilizing the COVID-19 Screening Tool, in advance, each day that they will have direct patient contact and in accordance with their person or persons who coordinate COVID-19 workplace safety efforts at covered contractor workplaces. Contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information. Contractor must immediately notify their COR or Contracting Officer if contract performance is jeopardized due to contractor employees being denied entry into VA Facilities. For indefinite delivery contracts: Contractor agrees to comply with VHA Supplemental Contract Requirements for any task or delivery orders issued prior to this modification when performance has already commenced.
