PROPOSAL FOR A SILVER STAR MAINTENANCE CONTRACT
TERM OF COVERAGE: July 1, 2022 - June 30, 2023
SILVER STAR FULL MAINTENANCE SERVICES PROVIDED:
1. Priority scheduling for all service
2. A Certified technician will respond during the hours of 8:00 am and 4:30 pm,
Monday - Friday.
PLEASE NOTE: Requests for service received after 2:00 pm will be
addressed the following morning. Emergency service
available at prevailing rates.
3. Coverage includes all labor and replacement parts
** For Incubators - Coverage includes all replacement parts with the
EXCEPTION of UV bulbs, HEPA, CO2 or In line filters.
4. Two (2) Preventive Maintenance Inspections will be performed
over the term of this contract for refrigerator and freezers
One (1) Preventive Maintenance Inspection will be performed over the
term of this contract - Incubators
5. Calibration/Inspection labels will be affixed to each unit at time of
each inspection.
6. After each Preventative Maintenance visit, customer will receive a PM
Packet to include: Certificates of Preventive Maintenance, Calibration
Certificates and an Equipment Status Report providing an overview of
our inspection findings.
