REQUEST FOR INFORMATION / SOURCES SOUGHT
DEPARTMENT OF VETERANS AFFAIRS, VISN 23 Sources Sought: The VA facilities in VISN 23 are looking
for vendors that are authorized distributors of the products listed below. We have immediate need for
these items and are asking vendors with quantities on hand to respond for consideration. Please include
in your response documentation that you are an authorized distributor of this product from the
manufacturer. Responses that are not from an authorized distributor will not be considered.
INTERESTED SOURCES SHALL RESPOND BY:
Please submit response no later than 3:00pm CST Thursday, April 21, 2022
All responses and questions must be emailed to Joseph.Bennett@va.gov and must include in the subject line: Sources Sought, B&D Butterfly Needles
POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE:
1. Company name, address, phone number, primary contact(s), e-mail address.
2. State which of these Brand Name items your firm distributes.
3. SAM Unique Entity Identifier.
4. NAICS (North American Industry Classification System) code that your firm uses for butterfly needles.
5. State if your firm is a large business, small business, service-disabled-veteran-owned
small business, veteran-owned small business, HUB zone 8(a), women-owned small business or a small-disadvantaged business.
6. State if your firm has the required items on Schedule (FSS, GSA, other…).
7. A letter, or other document, confirming that your firm is an authorized distributor of the manufacturer from the
manufacturer. Grey goods are not acceptable.
NOTE: Responses that are not from an authorized distributor will not be considered.
8. State if your firm has the required quantities on hand and can respond to a potential
solicitation in the near future.
