Specifications include, but are not limited to: Contractor shall furnish all labor, materials, etc. necessary to carry out furnishing and delivery/placement of the proposed oil/water separators at various locations statewide under a master agreement all in accordance with the plans and specifications. If the oil/water separator has a holding tank (greater than 110 gallons) for the oil collected, the contractor installing the tanks must be certified by the Kentucky State Fire Marshal’s Office. Contractors that are not currently on the list but wish to submit a bid for this solicitation must be registered prior to the bid closing date. All vendors should submit proof of certification with their bid submission. Vendors using subcontractors for any part of this contract shall complete and submit Attachment C Required Affidavit Regarding Subcontractors listing all intended subcontractors. Awarded vendor shall notify the Division of Purchases Buyer listed below of any changes or additions to the listed contractors within two (2) days prior to performing any services. Services shall not be provided by any subcontractor unless written approval has been received by the Division of Purchases.
