Specifications include, but are not limited to: 1. The Offeror must provide services that reduce recidivism and divert youth from further involvement in the juvenile justice system through early intervention and assessment in Sandoval County at various sites throughout Sandoval County, NM as agreed upon and determined by the Sandoval County Juvenile Justice Advisory Board to include community based sites in rural areas and pueblos. The following are required to be completed on each referred youth: a. An intake interview with youth to be completed within three to seven (3 to 7) business days of referral to identify risk factors. b. A needs assessment, using an evidence based tool agreed upon Sandoval County Juvenile Justice Advisory Board as well as the Juvenile Inventory for Functioning (JIFF) to assess needs of the youth within three (3) business days of intake interview. c. On-going case management which includes maintaining contact with youth for a minimum of 45 days to follow up on grades, attendance and the overall risk needs of the youth. 2. The Offeror shall perform these services at Bernalillo Public Schools, Rio Rancho Public Schools, and community based sites in rural areas and pueblos as identified by and approved by Sandoval County Juvenile Justice Advisory Board. a. The Offeror must provide three (3) full-time employees and transportation to make available mobile services in community locations throughout Sandoval County, including rural areas and pueblos. b. The Offeror shall provide services at the determined locations 250 weekdays (Monday to Friday). c. The Offeror shall identify students to participate in the Sandoval County Youth Council and aid in transportation and arrangements to participate in Sandoval County Juvenile Justice Advisory Board meetings. 3. The Offeror will assist families in accessing services and making appointments as needed. 4. The Offeror will provide a monthly half page summary recognizing interventions, trends, customs/observed holidays identified for the current and upcoming months as well as report on accomplishments and challenges of the program to the Juvenile Continuum Coordinator as well as attend scheduled quarterly meetings. 5. The Offeror will submit monthly request for youth registry numbers to Juvenile Continuum Coordinator by last business date of the month.
