In addition, on-going consulting and training Services are necessary for ensuring that the State and local HFO Program Providers are equipped with the knowledge and skills necessary to achieving the goals of the HFO Program. Accordingly, this RFP is intended to identify a contractor to complete the annual evaluation of the HFO Program and to provide the consulting and training Services that will assist the State and local HFO Program providers meet their obligations to the children and families they serve. • Project kickoff meeting • Data analysis and reporting • Project management and reporting • Re-evaluation of the New Baby Questionnaire (NBQ) • Evaluation of eligibility criteria • Annual Implementation Plan for completing annual evaluation reports, including detailed information about how data elements will be required, combined and analyzed.
