The Maine CDC is charged with providing leadership, expertise, information, and tools assuring all Maine residents have the access and ability to live healthy lives. The goal of the Maine CDC Chronic Disease Prevention and Control Program is to implement evidence-based programs for chronic disease prevention, by combining environmental approaches, collaborating with health care systems, and developing clinical and community linkages. The Maine CDC Chronic Disease Prevention and Control Program develops and applies collaborative strategies to improve the health of Maine people by: Mobilizing partners to support evidence-based prevention efforts; Gathering and disseminating data to drive healthy decision making; Empowering patients to self-manage chronic disease; Promoting evidence-based guidelines to strengthen health care systems; Linking health care systems with community resources; Increasing access to screening for early detectable chronic diseases; and Promoting evidence-based screening recommendations.
