Specifications include, but are not limited to: Phase One โ Conduct a centralized intake pilot to determine viability, The contractor in close collaboration with the State, will design and implement a centralized intake pilot for the DVR program to evaluate the effectiveness of the model. The State anticipates after going live, the pilot will run for three (3) months with the possibility of an additional three (3) month extension. The pilot will include: a. A target number of referrals to be processed during the pilot to ensure a valid test of viability. b. The contractor must process an eligibility recommendation for all referrals to the pilot as outlined in Section 2. c. An evaluation of the effectiveness of the pilot to be conducted by the State. At the end of the pilot phase the State will determine if it wants to proceed with Statewide implementation of centralized intake with the contractor. Phase Two โ Implement a statewide Centralized Intake process. Contractor shall perform the following service: a. Process and complete an initial interview, an Eligibility/Order of Selection (OOS) recommendation for up to 2,000 referrals annually. b. Provide completed initial interview, and Eligibility/OOS recommendation and within 30 days of receipt of each referral. c. Maintain the confidentiality of all information transmitted from DVR for each referral/applicant. Contractor must not release Personally Identifiable Information (PII) or records regarding any participant authorized for services by DVR without the prior written consent of the participant. Additionally, contractor must safeguard all participant personal information and records to prevent the inadvertent or unintentional disclosures. Contractor must comply with all confidentiality requirements in accordance with Health Insurance Portability and Accountability Act (HIPAA) and Rehab Act regulations ( The Rehabilitation Act of 1973 as amended by title IV of WIOA (PDF 791KB) ) d. Conduct comprehensive initial interview, and Eligibility/OOS assessment for the person applying for services to include but not be limited to exploration of current and past work history, education history, impact of applicant's disability and resulting functional impairments as they relate to obtaining, maintaining, and advancing in employment. e. Collect and review medical records and/or other supporting documentation as directed by the applicant and/or referring person to support information shared through selfreporting at time of Eligibility/OOS assessment to assess and verify the existence of a physical or mental impairment and the impact to employment for the person applying for DVR services. f. Draft and submit the Eligibility/OOS worksheet to provide eligibility recommendations to the DVR vocational counselor.
