Specifications include, but are not limited to: CDD is requesting applications from qualified organizations that will work closely with the State of Vermont to carry out projects that strengthen family engagement, inclusive of families with children ages 0 - 3 who are experiencing or are suspected of having developmental delays or diagnosed conditions that may result in developmental delays, at the regional level by addressing at least one of four project goals: • Provide families with opportunities to strengthen their social networks by meeting other families with young children; • Provide families with opportunities to learn about CIS services and supports; • Provide families opportunities to develop leadership skills; and/or • Provide families opportunities to partner in CIS continuous quality improvement work. 1. Implement a regional project designed to strengthen family engagement in response to needs identified in individual regional planning process that addresses one or more of the four goals/outcomes listed above in C. SERVICE GOALS & OUTCOMES. Projects shall be inclusive of families with children ages 0 - 3 who are experiencing or are suspected of having developmental delays or diagnosed conditions that may result in developmental delays. 2. Revise and implement evaluation plan, and submit Evaluation Plan Results (Form B) that measures the ways and level to which family engagement was strengthened in the Subrecipient’s respective region no later than the award end date. a. Evaluation shall include how family voice informed the project and its implementation. b. Evaluation shall include how and to what level the project improved targeted family engagement as related to the project’s identified goal area. 3. Complete Performance Monitoring Reports by specified dates, contact the CDD Program Manager with any questions, or updates on the grant specifications or performance monitoring questions immediately as they occur within the grant term.
