Specifications include, but are not limited to: The New Hampshire Department of Health and Human Services, Division of Long Term Supports and Services (DLTSS) (“Department”) is seeking responses to this Request for Proposals (solicitation) from qualified Vendors to develop and conduct a statewide assessment of case management and training needs among stakeholders for the Division of Long Term Supports and Services (DLTSS), including, but not limited to, the Bureau of Developmental Services Area Agency system (AAs), and Independent Case Management Agencies (ICMAs). This will include developing curriculum and a training program, in order to provide training to agency staff who perform Home and Community Based Services Waiver service coordination and case management throughout the state. Services provided through the resulting contract will address ongoing staff turnover, and inconsistent approaches to case management delivery, by creating consistency in service provision and clear expectations about staff roles and responsibilities. This will be attained by developing training materials and providing evidence-based trainings which give all case management professionals foundational knowledge.
