Specifications include, but are not limited to: • Provide approximately 750 whole body badges exchanged quarterly, and 250 whole body badges exchanged monthly. Currently using TLD. • Provide approximately 150 extremity monitors (ring, wrist etc.) exchanged quarterly, and 30 exchanged monthly. • Provide approximately 125 neutron monitoring badges. • Provide 20 area neutron monitors exchanged quarterly, and 10 regular area monitors. • Series includes ~ 50 quarterly and 30 monthly. • SRMC has < 20 WB badges exchanged quarterly and < 10 rings. • Fetal badges as requested. • Annual Form 5's – duplicate copies • Absentee reporting • Web access for dosimetry reports and managing changes (new and terminating participants). Paper copies to be sent. (2 copies required) Ability to secure personal information on both electronic and paper copies. • Alerts for badges exceeding preset dose levels. • Bar code badge tracking capability. • Provide on site demonstration of new products and services to best serve our account. • The vendor selected shall be accredited by the National Voluntary Lab Accreditation Program (NAVLAP).
