Specifications include, but are not limited to: Vendor must provide a licensed Psychologist on site for a maximum of twelve (12) hours per week, to evaluate residents with a behavioral problem or mental illness upon admission to determine if a plan needs developed to address any behavioral issues. Psychologist must evaluate/review each resident with a plan to address behavioral issues monthly. (Max of 114 residents, 70 residents is average census, approximately five (5) new admissions per month, and no more than ten (10) total behavior plans at a time. Psychologist must assist providing staff education as needed for resident psychology related concerns/ communication. Psychologist must document in the resident ‘record at the time services is rendered. Psychologist must participate in care conference meetings as needed and meet with multi-disciplinary team monthly to review all resident behavioral plans. Other services to include determining competency/capacity, guardianship/conservatorship, and counseling for resident. Psychologist hours of service not to exceed 624 hours within a twelve (12) month period. Psychologist can choose which day they work as long as it’s Monday thru Friday 8am to 4pm excluding any legal holidays. Any anticipated travel must be incorporated in the vendor’s fee. No travel will be reimbursed by the State and is the sole responsibility of the vendor. Vendor must assume the responsibility for the billing of all services provided to the insurance via Medicare, Medicaid, or third-party insurances. This information will be provided to the vendor by the facility. The vendor must hold the facility harmless in any cases of non-collection.
