Specifications include, but are not limited to: The purpose of this solicitation is to procure a Community Based Intensive Care team(s) to address a critical systems gap: a lack of intensive, home-based treatment that can provide outreach, case management, and day treatment in the home for extended hours, as well as afterhours support for children and their families who face persistent, complex mental health struggles and who need more care than the existing home and community-based services array can provide. Service Criteria Framework - The CBIC Team should: Serve children and youth between 5 and 21 who have been diagnosed with SED and who require intensive home-based services for their treatment than currently available through home-based care. Use a broad service approach with the youth’s caregiver(s) – parents, grandparents, foster parents, etc. – who face challenges such as mental illness, substance use, a history of trauma, abuse, or neglect, or cognitive limitations. Provide a set of trauma-informed, evidence-based services that can include the following in the family’s home: individual therapy, family therapy, skills training, specialized consultation and group therapy, psychoeducational groups, and intensive care coordination and natural support building, with all of these supports and crisis response available 24/7, when needed. Include involvement with other family and natural supports, as well as schools, community resources, and service systems. Address the social determinants of health needs of families in its care.
