Specifications include, but are not limited to: The City of Sioux Falls seeks a Homelessness Street Outreach Services provider that can provide the following minimum services: 1.) Proactive on-the-street outreach is essential to building trust and respect between homeless clients and agencies. Street outreach is also a key component of identifying the best means to assist clients find more permanent services and treatment plans suited to their needs. The proposer shall perform the following tasks listed below: a. Minimum 30 hours per week of active intervention coverage in designated areas identified by the City. b. Implement best practices designed to engage homeless individuals in a productive, compassionate manner. c. Utilize social work best practices related to serving clients experiencing various barriers including, but not limited to, people suffering from mental health and/or addiction issues, people who have a history in the court system, and past trauma. d. Build rapport and positive relationships with homeless individuals to provide linkages to housing, treatment, and other supportive services. e. Identify and conduct active intervention in areas where homeless people live and engage in activities during the day, such as encampments, under bridges, streets, and isolated areas. f. Work with and refer individuals and families to the full suite of services in the continuum of care, including behavioral health, physical health, addiction treatment, and applicable training/developmental programs. g. Collaborate with key stakeholders and providers to create and implement plans to address gaps in services. h. Identify immediate needs that require short-term interventions and/or emergency services. i. Document all interactions in the Helpline Network of Care database. j. Utilize appropriate, clearly marked vehicle that can accommodate multiple passengers of varying degrees of mobility.
