Specifications include, but are not limited to: In Phase 1 (1-2 months), the Contractor will assist the Community of Practice for CLC in selecting three regional teams and develop a half-day “kick-off” event for the selected Teams. The Regional Teams will be centered at designated and/or specialized services agencies for Developmental Disabilities Services, but they may incorporate other local partners. (For example, school transition coordinators, early childhood educators, home health providers, or mental health providers who serve the I/DD population). Each Regional Team will receive a “mini grant” of $10,000 to cover their costs, including stipends for community partners to attend meetings. In Phase 2 (10-11 months), the Contractor will coach the Regional Teams through a series of tasks, meeting with the Teams individually or collectively as time and budget allow, but minimally once a month. There will be three quarterly plenary sessions that bring together the Community of Practice for CLC and all three Regional Teams. The Consultant will plan and facilitate these plenary sessions with the Community of Practice for CLC.
