Specifications include, but are not limited to: Doña Ana County (County) is soliciting competitive sealed proposals from qualified Offerors to provide Access to End-of-Life / Hospice Care for qualified indigent residents of the County; The Offeror shall provide services throughout rural communities in Doña Ana County. The County is seeking providers to provide end-of-life/hospice care services for qualified indigent residents of Doña Ana County in accordance with national, state, and local policy. The agency must operate in accordance with all professional standards and sustain good standing within the state of New Mexico adhering to all regulations and standards including but not limited to: New Mexico Department of Health, Public Health Division, Health Facility Licensing and Certification Bureau 7.12.2 NMAC; Health Facility Licensure Fees and Procedures, New Mexico Department of Health - 7.1.7 NMAC; Health Facility Sanctions and Civil Monetary Penalties, New Mexico Department of Health - 7.1.8 NMAC; and Adjudicatory Hearings, New Mexico Department of Health - 7.1.2 NMAC. End-of-life/hospice care services should include, but not be limited to: Available medically necessary health care services for people diagnosed with an illness that limits their life expectancy. Services rendered in the patient’s home or licensed facility (e.g. hospital, nursing home, assisted living facility or other licensed residential facility) Patients with levels of care that range from routine (palliative care) to higher acuity (care for pain control and/or acute or chronic symptom management) from an interdisciplinary care team. Bereavement care services to assist family members through the grieving process.
