Specifications include, but are not limited to: Doña Ana County is soliciting competitive sealed proposals from qualified Offerors to provide Intensive Case Management for adults involved or at risk of involvement in the criminal justice system in accordance with the Scope of Work specified in this Request for Proposal (RFP). According to the National Association of Social Workers the goal of case management is to “enhance a client’s purpose and well-being by providing and coordinating high-quality, services, in the most effective and efficient manner possible, for individuals with multiple complex needs. As the County and its partners implement diversion programs, it is evident that case management for adults who are essential to serve is critical to successful program outcomes and an improved quality of life for all residents. 1. Peer Engagement: Creatively and effectively engage with people who may have a history of non-engagement and understand that this population is essential to engage and not difficult to engage. 2. Screening: Initial check in with all clients to determine needs, level of interest in available services, and eligibility for different programs and services. 3. Assessment: Gather information from clients to help case manager determine any issues the client may be struggling with and how these issues or behaviors are impacting various aspects of their lives. 4. Planning: Develop an individualized care plan with client utilizing the screening and assessment. 5. Care Coordination/Navigation: Connect client to a wide array of needed services, meeting the client, and making appropriate treatment plans based in their needs. 6. Connect with LEAD or other participants at least once a week.
