The ink must be: 287 Blue (Face and Back) Paper, heat resistant on front and back: 24# White OCR Perforations: Standard Specialty Business Paper-Medical cards forms must be manufactured to be printed on Ricoh Pro C9210 Graphics Arts Printer, Konica Minolta Accurio Press 6136P and Kodak Digimaster 125EX printers. Specialty Business Paper-Medical cards must be thoroughly vacuumed to remove paper dust. Specialty Business Paper-Medical card forms must be packaged and shrink wrapped 2000 sheets per box/carton. Vendor must provide a total quantity of 250,000 Specialty Business Paper-Medical card forms, annually, Lasermate Pressure Seal format or equal. The boxes/cartons stacked on a pallet must be poly-wrapped together as a unit. The poly-wrap must encompass the sides, top and bottom to form a complete moisture barrier. The cartons must not be openly exposed once they have been wrapped on the pallet. The individual Specialty Business Paper- Medical Card is produced on 24# pressure seal paper. Specialty Business Paper-Medical Cards color is solid blue with screened percentages to make the color appear differently. Specialty Business Paper-Medical Cards must be provided as displayed in attachment to this solicitation labeled Medical Card Example and Medical Card Example 2. The “x” on the examples must be deleted. Card needs to be able to be torn out.
