Specifications include, but are not limited to: Through this Request for Proposal (RFP) the Agency of Human Services (hereinafter the“State”) is seeking to establish a contract with a vendor to provide technical assistance in the planning,development, implementation, and oversight of Vermont’s Home and Community-Based Services(HCBS) Grant Program funded through Section 9817 of the American Rescue Plan Act of 2021.Technical assistance (TA) will include: A.Grant Program Design: In collaboration with the State, the vendor will conduct stakeholder engagement meetings and design a grant program that 1) addresses the needs of HCBS providers, community-based organizations serving HCBS populations, and individuals and families who utilizeHCBS, 2) aligns with the State’s priorities, and 3) meets federal requirements for Section 9817funding.B.Application Materials Development and Roll Out: The vendor will develop application materialsand manage outreach to eligible organizations. C. Application Management and Evaluation: The vendor will organize and participate in the reviewof applications and contribute to final recommendations on grant awards.D.Grant Program Launch and Management:The vendor will serve as the primary point of contactfor the awardees and provide program oversight to ensure successful implementation.
