Specifications include, but are not limited to: The CDPHPD seeks providers that can build on the accomplishments achieved by the DP18-1815 (Category A) cooperative agreement. Respondents to this RFI should focus on strategies where they have capacity, subject matter expertise, and potential to achieve greatest reach and impact. Where appropriate, respondents are encouraged to apply their selected strategies in the same targeted communities and/or settings so that work in the strategies may be mutually reinforcing. Evidence-based Approaches to Diabetes Management Strategies Improve access, appropriateness, and feasibility of Diabetes Self-Management Education and Support (“DSMES”) services. Prevent diabetes complications through early detection (including diabetic retinopathy screening and early detection of chronic kidney disease). Improve acceptability and quality of diabetes care for priority populations (includes team-based care approaches and efforts to use clinical systems and care practices).
