Specifications include, but are not limited to: Doña Ana County (DAC) is soliciting competitive sealed proposals from qualified Offerors to provide Transitional Housing for the Doña Ana County Law Enforcement Assisted Diversion (LEAD) participants ; The Offeror shall provide community based transitional housing assistance through a collaborative partnership with the criminal justice system and Doña Ana County through the Law Enforcement Assisted Diversion (LEAD) Program. The Offeror shall also provide community-based support services for the Assisted Outpatient Treatment (AOT) Program. The Assisted Outpatient Treatment (AOT) Program utilizes evidence-based practices to reduce the incidence of duration of psychiatric hospitalization, homelessness, incarcerations, and interactions with the criminal justice system while improving outcomes of individuals with a serious mental illness. 1. Obtain housing for eligible LEAD participants. 2. Provide Emergency Housing for eligible LEAD participants. This will include covering and providing lodging, food, hygiene items, and clothes. 3. Receive referrals from the LEAD team members, participate in team meetings, and coordinate with treatment staff to avoid duplicate efforts. 4. Participate in reporting and evaluation of the LEAD program. 5. Provide case management through the Housing Navigator to qualified LEAD participants. Case management to include support with identification, birth certificates, pre-paid phones, tenancy support, housing support, housing payments, home visits, transportation, income support, crisis intervention, and additional supportive services that will lead to successful participation in the program and quality of life improvement to LEAD participants. 6. Participate in the LEAD Program, as a team member, and assist with coordinating access to housing and other social supports to program participants.
