Specifications include, but are not limited to: Provide confidential counseling, advice, and support to HSH’s patients and their families relative to their spiritual and religious well-being which may include, but not be limited to, the following: a. Upon request by the HSH, provide for religious services at the HSH and for patients discharged and moved into the community. Services shall also be provided to STATE's Community Mental Health Center ("CENTER") patients who need counseling, advice, and support as part of their continuum of care. b. Be available, at the discretion of the HSH staff, to assist with the emotional and religious needs of HSH’s patients and their families. c. Follow-up counseling, support, and emergency crisis counseling services as determined by HSH staff. 2. Assign two (2) full-time Institutional Chaplains that meet the Common Certification Standards for Professional Chaplains to manage all HSH ministry activities. The Institutional Chaplains shall ensure that all ministry services for the HSH's patients and their families are consistent with the guidelines set forth in the Standards for Accredited Chaplaincy Services published by the Congress on Ministry in Specialized Settings, 2012. The Institutional Chaplains shall receive supervision from the HOSPITAL's Psychosocial Rehabilitation Specialist. The Institutional Chaplains' duties shall include, but not be limited to, the following: a. Assisting in case review and treatment planning for HSH patients, upon request by HSH staff; b. Reporting on a regular basis to the HSH's Administrator on HSH ministry activities; c. Conducting training programs, seminars, and group discussions for HSH staff in conjunction with other HSH staff development programs; d. Providing consultation for the HSH's staff concerning the HSH patients' religious needs
