Specifications include, but are not limited to: The Idaho Department of Health and Welfare, Division of Public Health (Department) is requesting quotes for providing service and repair of electric embossing sealers used by The Idaho Bureau of Vital Records and Health Statistics (BVRHS) located at 450 W State Street, Boise, ID 83702; A. The Contractor must provide pick-up/drop-off services for the repair and service of twelve (12) Justrite JES-110 Heavy Duty Embosser sealers, or similar machines used by BVRHS and the seven (7) Shiny, model number 27-43963, Heavy Duty handheld embosser sealers used by PHD. Pick-up/drop-off service is located at 450 W State Street, Boise, ID 83702. 1. The Contractor must pick up Embossers for service and or repair within (five) 5 business days once the Department has submitted a request and must: 2. The Contractor must provide an estimated service and or repair time frame to the Department including drop off date. 3. Services and or Repair provided must include but not be limited to: a. Replacement of release springs; b. Replacement of die holders; c. Replacement of die transfers; d. Recast upper and lower seal dies; and e. Create a new die cast. B. If the Contractor is unable to service and or repair the Embosser, the Contractor may, with Department approval ship the Embossers to a vendor of your choice for service and or repair. 1. If a vendor, other than the Contractor, will be utilized to perform the service or repair, they must be an authorized technician for service and repair by Justrite and or Shiny.
