Specifications include, but are not limited to: Pre-Release Services: Meet with pre-selected potential participants at MDOC’s state prisons, regional facilities and private prisons to promote the program and obtain consents to participate Utilize clinical assessment to evaluate offenders for COD and appropriateness of program approximately 90-120 days pre-release, and incorporate MDOC risk assessment and criminogenic needs assessment to develop risk-informed individual service plans Initiate recovery support services (housing, medical, vocational/educational, and faithbased services, etc.) Post-Release Services: CMHP shall provide services to offenders deemed eligible for the program and returning to Panola, Lafayette, Marshall, Tate, Calhoun, and Yalobusha counties Outpatient services provided in accordance with MDMH Operational Standards Intensive outpatient services provided in accordance with MDMH Operational Standards Recovery support services consisting of linkage to housing, medical, vocational/educational, and faith-based services according to MDMH Operational Standards Collaboration with MDOC case managers, and Community Corrections staff regarding development of comprehensive case plans for each participant based on the risk level and criminogenic needs identified with the MDOC risk tool and needs survey, and the specific COD diagnosis and symptoms resulting from the COD screening and assessments
