Specifications include, but are not limited to: Through this Request for Proposal (RFP) the Office of Purchasing & Contracting Contracting on behalf of the Agency of Human Services, Vermont Department of Health (hereinafter the “State”) is seeking to establish contracts with one or more companies that can provide sealable, liquid resistant, 8 x 11-inch preaddressed prescription drug mail-back envelopes upon purchase by the State. The purchase of the envelopes will be scheduled for an “as needed” basis. Envelopes and method of medication destruction must comply with DEA standards. The destruction of said envelopes should include; the acceptance of the mail-back envelopes (including any controlled substances therein) and the incineration of all mail-back envelopes and contents. The Vendor shall provide monthly reports following the acceptance/destruction of mail back envelopes.
