Specifications include, but are not limited to: This RFP is for the augmentation of Crew 916 on I-515 mainline 3R/Capacity project with spot slab replacements, spall repair, and median barrier replacement. The project also includes new median lighting and drainage improvements. The SERVICE PROVIDER shall provide a principal engineer to act as Project Manager. The Principal Engineer shall be limited to billing no more than eight (8) hours per month unless prior approval for additional hours is obtained from the DEPARTMENT. The SERVICE PROVIDER shall provide one (1) Office Manager*, two (2) Level IV Inspectors, one (1) Level III Inspector, one (1) Level I Inspector, (4) Level III Testers, and a two (2)-person Survey Team. The SERVICE PROVIDER will also provide four (4) nuclear gauges, appropriate vehicles, iPads, and cell phones and proper safety equipment for all personnel assigned to this project; * denotes “Key Personnel”. The SERVICE PROVIDER shall provide incidental equipment as may be required by the DEPARTMENT. The SERVICE PROVIDER shall provide all personnel assigned to this project the proper safety equipment, including but not limited to soft caps, hard hats and vests meeting the current DEPARTMENT standards for Work Zone Apparel.
