Specifications include, but are not limited to: 1. Prospective contractors must be appropriately licensed and regularly engaged in the business of transporting and recycling fluorescent and HID lamps as well as ballast, household batteries and mercury-containing devices. Contractors shall provide applicable license and / or permit verification with the bid. 2. The contractor shall provide all labor, materials and equipment necessary for the proper containment, handling, loading, transportation, testing, treatment and disposal of materials and these will be included in the bid pricing. INDOT will collect and store the items in the appropriate shipping containers at specified locations prior to contractor removal. 3. Pricing is on a “per container” or volume and not weight basis for each type of material included on the attached bid forms. 4. The bidder is encouraged to provide INDOT with optional pricing for other types of lamps (not listed) under the same general terms and conditions. This pricing should preferably be on a volume or per container basis.
