Specifications include, but are not limited to: The goal of this project is to strengthen the referral linkages across evidence-basedhome visiting programs and community health worker agencies. In doing so, familiescurrently enrolled in evidence-based home visiting programs who have unmet adversesocial and structural determinants of health that cannot be readily nor robustlyaddressed by home visitors will be referred to community health workers who have thecapacity and capability to assist these families. Correspondingly, families who haveworked with community health worker agencies and who are eligible yet not enrolled inevidence-based home visiting due to limited familiarity of such programs will be referredto such programs by their respective community health worker agency.The Home Visiting Community Health Worker Project hopes to (1) increase therecruitment of eligible families who are historically unserved by home visiting or who facedisproportionate barriers to accessing or participating in services and (2) advance theoverall population health and improvement in health equity within the State of Delaware.Objectives include the development of MOUs for data sharing across home visitingprograms and community health agencies, the inclusion of home visiting - eligible criteriain community health worker forms, the inclusion of community health worker referralnotation in home visiting intake and post-enrollment forms, the creation of quarterlydashboards, and the carrying out of continuous quality improvement efforts
