Specifications include, but are not limited to: 1. General Requirements a. Collaborate with the Demonstration Site to complete the activities of the Demonstration as outlined in RFP# 202212190 R-CHIP Component A: Demonstration Project – Phase 1. b. Assist the Demonstration Site in completing a Readiness Assessment, including: i. A description and assessment of community health needs and priorities; ii. A description of existing community-level or regional collaboration(s) pertaining to Health-Related Social Need (HRSNs), including gaps and opportunities for strengthening the Community Partnership; iii. A description of current health care, public health, and social service funding and service delivery arrangements, identifying barriers to addressing HRSNs and opportunities for improvement; iv. An assessment of the adequacy and appropriateness of the current system of care for identifying and addressing HRSNs in the community, identifying gaps, barriers, and opportunities for improvement; v. A description and assessment of HRSN screening systems, cross system communications and information sharing concerning HRSNs, and opportunities for improvement; and vi. A description and assessment of existing workforce capacity and needs for addressing HRSNs in the community. c. Work with Demonstration Site on planning for implementation, including: i. Aligning sectors (community leadership and collaboration); ii. Strategies and steps to operationalizing (mechanisms for identifying social needs across sectors); iii. Workforce; iv. Data and digital tools and strategies; and v. Funding and financial systems. d. Work with the Demonstration Site on sustainability planning for Phase 2 of the Demonstration project. e. Function as a distributed resource, engaging individuals and organizations as consultants or advisors on an ongoing or as-needed basis to address the Demonstration Site’s TA needs. i. Collaborate with the Demonstration Site and network partner(s) to identify TA resources, the nature of services or support to be provided, level of effort and timeline(s).
