Specifications include, but are not limited to: Primary Care Services for VPCH 2.4.1. Up to 0.5 FTE [20 hours per week] medical healthcare practitioner services, with the level of effort to be reviewed and adjusted based on the volume of work. In addition, should additional hours in excess of 0.5 FTE be needed, Contractor shall request an amendment to the agreement. 2.4.2. The responsibilities of the medical practitioners shall be as follows: 2.4.2.1. Provide history and physical assessments next business day to incoming patients to VPCH. 2.4.2.2. Provide physical healthcare patient consultations and follow-up as requested by VPCH providers. 2.4.2.3. Develop and implement patient wellness programming with VPCH staff. 2.4.2.4. Attend administrative and educational medical and hospital staff meetings as determined by the Executive Medical Officer. 2.4.2.5. Contractor may be asked to provide primary care services for DMH outside of the VPCH facility; however, it is anticipated that it would be a rare occurrence if community providers could not be utilized under certain circumstances. Key Components 2.5.3.1.1. Psychiatric evaluation, service, medication management and consultation 2.5.3.1.2. Trauma-responsive setting and treatment approaches 2.5.3.1.3. Family involved in treatment as clinically indicated 2.5.3.1.4. Discharge planning in collaboration with family, relevant community, and State partners 2.5.3.1.5. Attend weekly treatment team meetings 2.5.3.1.6. Review treatment plans, Order of Non-Hospitalization documents 2.5.3.1.7. Coordination of care with outside providers as needed 2.5.3.1.8. May need to attend court hearings as needed
