Specifications include, but are not limited to: To provide HIV/AIDS services to eligible populations in Delaware by achieving the following goals: • Reducing the number of new patients who are already AIDS-defined at the time they enter into care. • Increasing access to and utilization of primary health care by the underserved by establishing clinical services in community and faith-based organizations. • Providing quality health care to HIV clients in a culturally competent, patient centered, community-based setting. • Improving patient care through frequent routine monitoring and evaluation of carerelated activities and patient satisfaction. • Increasing patient understanding regarding their HIV infection and self-care. This is expected to increase client participation in health care decisions.
