Specifications include, but are not limited to: UNM currently offers flexible spending account (FSA) benefits to eligible employees. UNM invites qualified flexible benefit administrators to submit proposals for the services listed; The successful Service Provider will cooperate with UNM to customize information and documents, including plan and enrollment materials, letters to participants, etc., that will specifically identify (brand) the UNM employee plan. All usual services incident to the settlement of claims in accordance with plan provisions, including but not limited to the provision of appropriate forms, adjudication, technical review, non-discrimination testing, records retention, check and EOB issuance, resolution of claimant inquiries and payment discrepancies and claim appeal procedures. We are requesting quotes for FSA (Health care and Dependent Care). All fee quotes must be on a per participant per month basis. Fee guarantees for the initial 3 years and maximum rate caps for years beyond 3 are also requested. • Service Provider will be asked to assist with preparation of all communication materials. This includes detailed Benefit Summaries, Plan and Benefit Booklets. Service Provider will provide all communication materials timely to UNM for review prior to publishing and distribution of materials. All communications must be reviewed by UNM prior to publish and distribution. UNM will be posting Benefit Booklets on their intranet sites – paper copies will not be distributed to employees but should be available upon request. • Quotes should also include FSA and Premium Only Plan (POP) non-discrimination testing services • COBRA Administration will be handled by an independent COBRA Administrator.
