Specifications include, but are not limited to: The West Virginia Department of Health and Human Resources, Office of Health Facilities (OHF), Mildred Mitchell-Bateman Hospital (MMBH) located at 1530 Norway Avenue, Huntington WV 25705, is soliciting bids to secure pricing for a Concrete Sidewalk around Building #5 on our campus. The agency is located at 1530 Norway Avenue, Huntington, WV 25705. The Vendor must furnish all materials, labor, and equipment necessary to complete and protect all phases of the Construction Services. The Vendor must furnish any incidental work, materials, labor, and equipment that are necessary to complete the Construction Services, even if such incidental work is not explicitly included in the Project Plans. 13.1 CONCRETE SIDEWALK 13.1.1 This project shall consist of excavation and the removal of all existing materials to a grade necessary for new construction, furnishing and placing all necessary concrete. 13.1.2 Estimated thirteen (13) yds of concrete. 13.1.2.1 Actual measurements will be the responsibility of the bidding company. 13.1.3 Estimated thirty (30) tons of gravel. 13.1.3.1 Actual measurement will be the responsibility of the bidding company. 13.1.4 Sidewalk must have a maximum height of 3.5” 13.2 CONSTRUCTION METHODS 13.2.1 Must remove all surface debris from the entire sidewalk area in and around the front side of Building 5. 13.2.2 Must remove all surface debris from the sidewalk area in and around the project area on the back side of Building 5. 13.2.3 Must apply surface bonding agent on existing concrete slabs. 13.2.4 Must create or do modification of existing sidewalk. 13.2.5 Must form the slope of concrete to allow for drainage away from around the building. 13.2.6 Must provide, install, and compact gravel in as necessary. 13.2.7 Must use Georgia buggy utilization for the placement of concrete with a minimum of 4,000 pounds per square inch (PSI) air entrained with fiber concrete utilized throughout the entire pour.
