Specifications include, but are not limited to: Johnson County is seeking proposals to facilitate the continuation of this service in a manner which will be selfsustaining, without the need for further subsidy. It seems clear to all involved that a skilled care facility providing services to large numbers of Medicaid recipients is not likely to be self-sustaining in the future, and as such, proposals should include elements besides skilled care that can help support high quality care for as large of a population of Medicaid recipients as possible. • Management of a high-quality senior living operation • The continuation of high-quality services for the current residents of Evergreen Community • Operation of services on a sound, self-supporting financial basis, including a plan for budgeting of capital expenditures. Services to be considered include: o Assisted living and assisted living memory care (HCBS) o PACE day center services o Home health services o Meal and dining services o Independent living o Clinics for health care, dental, podiatry, and behavioral health • Adhering to, and fully complying with, all applicable State and federal rules and regulations • Maintaining high levels of customer/resident satisfaction
