Veteran’s Administration (VA) Service-Connected Rating, if any. 2.1.1.4. Enrolled in VA Healthcare 2.1.1.5. Member of a Veteran Service Organization VFW, American Legion, DAV, etc. 2.1.2.Identification of competitors throughout Vermont who are contracted with the VA and/or accept VA Optum(including location and number of Veterans served) providing the following services to Veterans in Vermont: 2.1.2.1. Skilled Nursing Care including, but not limited to, short-term rehab, long-term care, memory care, respite care, and Hospice/palliative care. 2.1.2.2. Home and community-based care including, but not limited to, Adult Day Health Care, visiting nurse services, Hospice, medical foster homes, assisted living, and residential care
