Specifications include, but are not limited to: The Office of Oil and Gas does have a partial well record and plugging permit that was issued in 1940. The well record is indexed under API 47-043-30220. The well record shows total depth (TD) of the well at 2172’, producing the Berea formation. The record shows 32’ of 10” conductor casing, 1670’ of 5-3/16” casing and 1800’ of 2 ⅜” tubing with packer set at 1700’. Casing string cement locations unknown. There is a gas show at 2080’ in the berea, gas show at 1435’ in the big lime and a gas show at 955’ in the salt sand. There is a coal show at 365’ and 268’, a freshwater show at 120’, and elevation at 713’. (See Appendix A for reference well record). V. REMEDIATION AND/OR PLUGGING WORK PLAN Contract work may require site remediation of the well site prior to the commencement of plugging operations. Project will commence, generally, as follows: 1. Pre-mobilization site remediation, if necessary, 2. Site preparation, 3. Connect rig to well, 4. Clean out well to total depth, 5. Set prescribed plugs and monument, 6. Reclaim site including removal of stream crossings, and 7. Submit invoice to DEP
