Specifications include, but are not limited to: The purpose of this solicitation is to procure a vendor to evaluate, recommend, and help implement a transformative support model that will improve long-term outcomes for adults with I/DD receiving services from DDD. • Hire professionals to provide advice/counseling to individuals on all aspects of what goes into self-directing one’s services. • Assist in shifting the system model to one driven by participant self-determination and control. • Provide ongoing staff training and development on topics like person centeredness, self determination, decision making, transitioning through life stages, technology, goods and services, dignity of/right to risk, Charting the Life Course, Circles of Support, and other topics related to navigating the self-direction service model. o Make trainings available in ASL and Spanish. • Provide mentoring and coaching to organization staff. • Assist with care coordination for both paid and natural supports. • Work/Collaborate with the State’s Fiscal Intermediaries licensed by BHDDH. his includes, but is not limited to the following: • Hire individuals who are self-directing their own services to provide advice/counseling to their peers on all aspects of what goes into self-directing one’s services. • Train the peer mentors so they are equipped with the tools and skill set to be successful peer mentors. • Provide ongoing support, training, and coaching to the peer mentors so they are able to succeed in their positions. • Provide ongoing staff training and development on topics like person centeredness, self determination, decision making, transitioning through life stages, technology, goods and services, dignity of/right to risk, and other topics related to navigating the self-direction service model.
