Specifications include, but are not limited to: Agency of Digital Services (hereinafter the “State”) is seeking to establish contracts with one or more companies that can provide a Salesforce solution that aligns with the existing statewide data model AND with the business requirements as described at a high-level here within. This project is limited to the Long-Term Care (LTC) area with the intent to utilize the Salesforce platform to create a single repository for LTC Medicaid client case tracking data. The solution will extract data from the State of Vermont (SoV) ACCESS mainframe system to auto-populate Salesforce client case records and combine it with client data input by LTC workers (e.g., resources, name of LTCCC (Long Term Care Clinical Coordinator). Further, this solution will utilize Salesforce functionality to pre-populate and auto generate many LTC Medicaid forms, including the LTC customer's annual Medicaid review form, using data stored in the client case record. The Salesforce generated forms will be stored (by clicking a button in Salesforce) in the SoV OnBase system. This project will use standard objects and features of the Salesforce Case Support Application so as not to impede future configuration, customization or component and feature reuse efforts. Note that the LTC team will continue to perform all eligibility functions in the SoV ACCESS mainframe system. Key outcomes of this project include implementation of a Salesforce solution with a period of support services including transition of support services to State of Vermont personnel over a period of up to two (2) years
