Specifications include, but are not limited to: The State of Delaware Department of Health and Social Services (DHSS), Division of Public Health (DPH) seeks professional services to provide Address Social Determinates of Health through Distribution of Resources. Goal 1: Ensure equitable access to critical resources available to address and support the social determinant of health needs of individuals by creating, establishing, and managing a funding mechanism that will house and disperse monetary assistance. The process must be developed in conjunction with the Delaware Division of Public Health’s (DPH) policies and procedures. 1.1 Identify a process for eligibility, application submission and aid distribution in conjunction with the Delaware Division of Public Health’s (DPH) policies and procedures. • Develop process and polices for administering, receiving, and awarding application requests. • Develop tracking system to monitor and report out on the distribution of resources. This may be done in an excel spreadsheet or the bidder may utilize other tools or resources that they already have in place that will allow them to demonstrate the type of aid distributed, the amount of aid distributed, the household income of the recipient, and the census tract the recipient resides in and other intake information. o Client Demographics o Client address- DPH is interested in reaching clients who reside in census tracts with a high social vulnerability index (SVI) o Client eligibility information o Resources provided; GOAL 2: Individuals working with a Community Health Worker who receive aid will have improved short, intermediate, and long-term health outcomes. 2.1 In collaboration with the Population Health Bureau’s Epidemiologists, evaluate the short, intermediate, and long-term outcomes of clients who utilize SDOH resources. 2.2 Provide information to legislators, key stakeholders, and community members on the benefits and challenges of providing SDOH funding to asset limited, income constrained, employed (ALICE) clients to inform future efforts. Data may include: • Information on hospital admissions or acute care utilization • Changes in client satisfaction • Outcomes of referrals • Adherence to preventative screenings • Outcomes of referrals • Other reportable challenges or successes • Impact on closing referral loops through the community pathway HUB;
