Specifications include, but are not limited to: Aging is seeking proposals from interested and qualified organizations and firms to administer Evidence Based Prevention Programming. The senior services programs serve older adults 60 years of age or older, with preference given to those in greatest economic or social need and to low-income, multi-ethnic individuals. The goals are to maintain or improve the physical, psychological, and social well-being of older individuals by providing appropriate senior services. Contractor shall perform all services in the manner prescribed by the scope of services description below: 1. Program Activities • Contractor will provide the management and coordination for the delivery of specified Evidence Based Prevention Programs for Milwaukee County older adults. Programs will include but not be limited to: Healthy Living with Diabetes, Healthy Living with Chronic Pain, Living Well with Chronic Conditions, Mind Over Matter, Physical Activity for Lifelong Success, Powerful Tools for Caregivers, Stand Up & Move More, Stepping On, and Walk With Ease. Management and coordination will include: o Management and coordination for the delivery of specified Evidence Based Prevention Programs for Milwaukee County older adults. o Facilitation of recruitment, scheduling, coordination, and evaluation of new classes, workshops and programs. o Facilitation of recruitment, training, scheduling, communication, and retention of new and current leaders. o Evaluate programs, as well as trained leaders, on an annual basis. • Contractor will attend regular meeting as prescribed by Community Programs Supervisor including: bi-monthly Milwaukee County Commission on Aging Wellness Committee meetings, monthly EBPP statewide meetings, Fall Prevention Coalition meetings, and Service Delivery Committee meetings. • Contractor will work with Community Programs Supervisor to achieve identified goals related to number of programs offered, number of trained leaders, and number of participants. • Contractor will coordinate this program with other community-based organizations which provide EBPP’s to older adults in Milwaukee County. • Contractor will make recommendations for future program offerings. • Contractor will partner with the Wisconsin Institute for Healthy Aging (WIHA) to provide facilitator trainings for the programs.
