Specifications include, but are not limited to: Complex Case Diagnosis and Evaluation Services Provide multidisciplinary evaluations to Clients referred by the Department and/or its agent by: Reviewing Client medical, behavioral, psychiatric, educational, and program or service records, including CDs/DVDs of studies or Client activities prior to the Client’s face-to-face examination. Reviewing specific surveys of events and symptoms provided in the referral packet. Initiating collateral calls and discussions with community team members, family, and caregivers prior to the Client’s scheduled face-to-face examination. Providing a direct brief office physical examination of the Client, as applicable. Providing a direct office neurological examination of the Client, as applicable. Ensuring a Psychiatrist and Psychologist who specializes in Intellectual Disabilities (ID) and has a valid in good standing license to practice in the State, interviews and observes the Client. Interviewing and discussing with caregivers and community team members attending the Client’s evaluation. Having an Evaluation Team discussion and review of the Client’s records/interviews. Developing a final report with summaries from each discipline identified in the Evaluation Team. Ensure the Evaluation Team performs a comprehensive evaluation that results in a detailed report, including a summary of a review of records, a summary assessment from each discipline leading to a case formulation, and recommendations from each discipline. Ensure the evaluation of Clients includes: Case approval and referral from the Department and initial consultation date and time. Referral packet and records that are obtained from the Client’s providers. The Evaluation Team Director: Conducting an initial call to family, case coordinators, or other informants to gain an overview of key clinical concerns. Reviewing referral packets for completeness and reporting missing elements and requesting additional information/documentation from the Department to be included in the Referral Packet. Conducting a detailed chart review by each individual identified in the Evaluation Team to include specific records and written reviews in preparation for evaluation day, two (2) weeks in advance. Ensuring the Client is called (either by the Team Director or his/her administrative assistant) to outline specific details of face-to-face evaluation. Coordinating completion of report components by each discipline. Distributing and reviewing with the Department and/or its agent, the final report developed by the Evaluation Team. A face-to-face evaluation with the Client, whenever possible. The Evaluation Team discussing findings and providing impressions/recommendations. Maintain records for all Client who receive complex case diagnosis and evaluation services with information relevant to all parts of the multidisciplinary evaluation, including but not limited to: Dates and times of each quarterly or ad hoc clinical consultation session; Whether the session was in-person or virtual; The Client case(s) reviewed; and The disposition of the case(s) reviewed. Clinical Case Consultation for OADS Crisis Team Provide sixteen (16), two (2) hour, in-person or virtual (audio-only conference calls are not permissible) case consultation sessions, annually, to the OADS Crisis Teams (one (1) session per team per quarter) which includes: An overview of Client cases, identified by the Department, to assist the Department by providing case review, recommendations, and strategies to best support the Clients. The Department will provide Clients’ case information two (2) weeks prior to each session. Provide ad hoc consultation sessions, as requested by the Department. Develop and administer a Department approved satisfaction survey to evaluate the effectiveness in providing case review, recommendations, and strategies to best support Clients which: Evaluates the confidence and skill level of the Crisis Team members after consultation when engaging with Clients with dual diagnosis of ID/Mental Health (MH). Is provided to Crisis Team members after each consultation session. Maintain records of all clinical case consultations, including but not limited to: Dates and times of each quarterly or ad hoc clinical case consultation session; Whether the session was in-person or by webinar; The client case(s) reviewed; and The disposition of the case(s) reviewed. Behavior Management Plan Development and Oversight Ensure services include all of the functions of a Qualified Professional to develop, oversee implementation of, and monitor Behavior Management Plans for individuals with ID, Autism Spectrum Disorder (ASD), and/or a Brain Injury (BI) as described in 14-197 C.M.R. Ch.5. Provide services throughout the State for clients who reside in a crisis house operated by the Department. Provide written guidance regarding the interpretation and implementation of Behavior Management Plans. Services shall include, but not be limited to: Participating in an initial meeting with the client and his/her Planning Team. Participating in the development of a client’s Behavior Management Plan. Overseeing implementation of a client’s Behavior Management Plan. Meeting and observing the client at least twice annually. Monitoring and documenting progress at least monthly. Providing monthly consultation to the Planning Team regarding the client. Maintain records of all Behavior Management Plan services provided which shall include information relevant to the requirements for Behavior Management Plans.
