Specifications include, but are not limited to: The Care Coordination Platform is a critical system to facilitate the delivery of services across multiple agencies and Delaware based organizations, caring for clients with Behavioral Health disorders. The application will provide visibility to all providers and help organizations identify, track, and better assist clients by removing the barriers of care around social, economic, and behavioral determinants of health. To that end, DSAMH is issuing a RFP for a solution that can break down the siloed systems of care that allows for and encourages effective and efficient care coordination for Behavioral Health clients in Delaware. Division of Substance Use and Mental Health wants to partner with a technology platform to create a robust care coordination Platform. The purpose of the Platform is to enhance care coordination, connect patients with health and social services providers, address social determinants of health (SDOH) needs, and deliver more integrated care to promote health and recovery. This RFP includes input from Delaware stakeholders and leverages existing State technologies to advance the State’s goals. It also considers stakeholders’ abilities to effectively utilize a state-wide care coordination platform.
