Specifications include, but are not limited to: A The Contractor shall provide, on a semiannual basis, all labor and material for a preventative maintenance service as specified herein for Boiler Control Systems. Work shall be bid as an annual lump sum amount. Work shall include providing Emergency Services at an additional charge to the contract. B Site Location and Representation: Daniel Cooper, Physical Plant Director Logansport State Hospital 1098 South State Road 25 Logansport, Indiana 46947-9699 574 737 3805 C The Contractor shall furnish all labor, materiel, and supervision required to maintain in optimum operating condition, each device of the Boiler Control Systems. Services shall include inspection, calibration, linkage and damper adjustment, cleaning, and lubrication, for all instruments, pertaining to the efficient operation of the system as listed on the Preventative Maintenance Checklist attached. D The minimum number of planned inspections shall be two (2) for one fiscal year for startup and shut down as coordinated by the Physical Plant Director. All systems must be operational prior to October 1st and shut down no earlier than May 1st . E Each preventative work period shall be prescheduled and directed from a maintenance management program initiated and maintained by the Contractor. The program may be computer directed, or manually operated. F All work shall be performed in the presence of an Agency Representative who shall sign documentation, provided by the Contractor, that the work is complete. The signatures of the foreman of the crew performing the work shall appear on all site prepared documents. G A report shall be furnished to the Physical Plant Director after each examination or Emergency Service indicating the scope of work performed, conditions requiring further attention and the name of the Agency Representative present. Failure to provide report may result in delay or lack of payment by owner.
