Specifications include, but are not limited to: The Minnesota Department of Health (MDH) requests proposals to provide COVID-19 vaccination in community clinic or Federally Qualified Health Center (FQHC) settings and in other community settings. Responders may propose vaccination services in A) community clinic or FQHC settings, B) other community settings, or both A and B. These services shall increase access to COVID-19 vaccination with a focus on communities of color, Indigenous communities, people with disabilities, refugee and immigrant communities, people experiencing homelessness, LGBTQ+ individuals, communities with high social vulnerability, and other historically underserved communities. Task A (Vaccination in a Community Clinic / FQHC): • Provide a brief overview of Responder’s background and experience, with examples of similar work done by Responder. • Provide a list of key personnel who will be responsible for leading the work carried out under this contract, including a brief description of their training and work experience. Do not include personal telephone numbers, home addresses, or home email addresses. • Provide the addresses of all the Responder’s clinic locations. • Describe which of these focus communities Responder serves: communities of color, Indigenous communities, people with disabilities, refugee and immigrant communities, people experiencing homelessness, LGBTQ+ individuals, communities with high social vulnerability, and other historically underserved communities. • Describe the demographics of the staff administering vaccinations. • Describe Responder’s efforts to help patients get all recommended doses of vaccine, such as providing phone or text reminders. • Describe Responder’s capability to report vaccination data, including demographic data on patients vaccinated (race/ethnicity, gender identity, age, zip code), weekly to MDH. • Provide the proposed total number of COVID-19 vaccinations to be administered within the community clinic or FQHC during the first year of the project, based on clinic capacity and typical numbers of patients. Proposed number of vaccinations administered in clinic during year 1; Task B (Vaccination in Other Community Settings): • Provide a brief overview of Responder’s background and experience, with examples of similar work done by Responder. • Describe experience with and capacity for mobile vaccination events (for example, at an encampment of people experiencing homelessness). • Provide a list of key personnel who will be responsible for leading the work carried out under this contract, including a brief description of their training and work experience. Do not include personal telephone numbers, home addresses, or home email addresses. • Describe the geographic area within Minnesota in which Responder is able to implement vaccination events (for example, the Twin Cities metro area, or the Arrowhead region of Northeast MN). • Describe which of these focus communities Responder serves: communities of color, Indigenous communities, people with disabilities, refugee and immigrant communities, people experiencing homelessness, LGBTQ+ individuals, communities with high social vulnerability, and other historically underserved communities. • Describe the demographics of the staff administering vaccinations. • Describe Responder’s efforts to help patients get all recommended doses of vaccine, such as providing phone or text reminders.
