Specifications include, but are not limited to: Referral The Contractor shall: 1. Accept all referrals made by the State, up to the referral cap per District Office served (Note to bidder: TBD, will be based on final Contract awards.). 2. Provide necessary screening services (i.e Family Finding, Family Safety Planning, and Family Time Coordination) to referred children, adolescents and their families following established protocols provided by the State. The screening shall include the completion of a Family Find; a service that gathers information from parent(s), known relatives, fictive kin, and others involved in the child’s life to expand the child’s network of support and create or maintain connections. Service Delivery The Contractor shall: 1. Provide Family Safety Planning Meetings within thirty (30) calendar days of referral date or as agreed upon between the Contractor and the referring local DCF District Office. Family Safety Planning Meetings provide parents, relatives, family friends, etc. a safe environment for discussion, problem solving, and planning to implement a collaborative approach in supporting the child. These meetings shall include: i. Notification by the Contractor around logistics associated with any such meetings; ii. Inclusion of members of the Child’s safety network in such meetings; iii. Neutral facilitation of meetings following safety organized practice guidelines established by the State; iv. Satisfaction surveys shall be provided to all attendees within fifteen (15) calendar days of each meeting held to evaluate if participants felt the meeting was helpful in moving the identified purpose forward ; and v. Provision of meeting notes to DCF Family Service Worker assigned to the case. 2. Provide Family Finding services to identify and grow natural supports around families within sixty (60) calendar days of referral date or as agreed upon between Contractor and the local DCF District Office including, but not limited to,: i. File mining; ii. Utilizing screening tools such as ecomaps, genograms, and family search engines; and iii. Reaching out to all family members identified to provide information around the current situation of the child and opportunities available for involvement in the child’s life. Direct contact through a telephone ca prior to other forms of contact whenever possible. 3. Formalize a parent-child contact plan within sixty (60) calendar days of referral date. Parent-child contact plans identify the safest and most appropriate form of contact (i.e. natural setting, in office, etc.) as well at the frequency at which contact between the parent(s) and Child(ren) shall occur. i. Parent-child contact that must occur during non-business hours shall be included in such plans as needed to align with family-friendly service delivery practices. 4. Complete a Child and Family Supports Plan of Care for each child referred by the State which documents child/family history, risks identified by the Family Services Worker, strengths of the family and child, goals developed by the parent(s), and indicators of progress. The Child and Family Supports Plan of Care shall be approved and monitored by a licensed practitioner of the healing arts on the Contractor’s staff as defined in the Policies and Procedures for Medicaid Rehabilitation Services within fifteen (15) business days of first contact with the child and/or family. The Plan of Care shall contain the following: i. Objectives which are measurable, realistic, and achievable; ii. A plan specifying services to be received, including level of services and projected timeframes for achievement of child and family goals; iii. Responsibilities of the treatment providers and family for the assigned tasks; and iv. A plan for coordinating services with other agencies, including coordination with the existing State Case Plan (a plan developed by the Family Services Worker in conjunction with the child(ren) and family that addresses unresolved danger or risk, identifies services to be provided to the family, and indicates the end goal for the child(ren) (i.e. reunification, adoption, etc.).
